Filing Requirements for Vendors

A person or business, and their agents who enter or seek to enter into a contract with the city for the sale or purchase of real property, goods, or services are required by Texas Local Government Code, Chapter 176, to complete and file a conflicts disclosure questionnaire (FORM CIQ) if the person has a business relationship with the governmental entity, and:

  1. Has an employment or other business relationship with an officer or family member of an officer; or,
  2. Has given an officer, or a family member of an officer, gifts with an aggregate value over $250.00 from such a person during the preceding 12 months.

The form is created by the Texas Ethics Commission and is available online at http://www.ethics.state.tx.us/whatsnew/conflict_forms.htm.  The completed Conflict of Interest Questionnaire Form CIQ must be filed with the city secretary no later than seven (7) days after the date the person or business begins discussions or negotiations with the city, or submits an application, response to a request for proposals or bids, correspondence, or other writing related to a potential contract with the city, or within 7 days of becoming aware of an employment or other business relationship or gifts to an officer or key employee, or family members of such persons, that would require disclosure.
